আবেদনপত্রের সাথে প্রার্থীকে (ক) জীবন-বৃত্তান্ত (খ) শিক্ষাগত যোগ্যতা ও অভিজ্ঞতার সদন (গ) সদ্য তোলা ০৩ কপি পাসপোর্ট সাইজের রঙ্গিন ছবি (ঘ) জাতীয় পরিচয় পত্র (ঙ) নাগরিকত্ব সদন (চ) প্রথম শ্রেণীর গেজেটেড কর্মকর্তা কর্তৃক প্রদত্ত রেফারেন্সসহ আগামী ২৮ শে জানুয়ারি ২০২০ তারিখের মধ্যে মহাপরিচালক, ওয়েজ আর্নার্স কল্যাণ বোর্ড বরাবর আবেদন করতে হবে।
আবেদনপত্রের সাথে আবেদনকারীর বর্তমান ও স্থায়ী ঠিকানা (ইংরেজি ও বাংলায়), মোবাইল নম্বর ও ই-মেইল আইডি উল্লেখ করতে হবে। মাধ্যমিক স্কুল সার্টিফিকেট পরীক্ষা (এস.এস.সি) বা সমমানের পরীক্ষায় সদনপত্রে উল্লেখিত জন্ম তারিখ অনুযায়ী প্রার্থীর বয়স নিয়োগ বিজ্ঞপ্তি প্রকাশের তারিখ উল্লেখিত বয়সসীমার মধ্যে হতে হবে। বয়সের ক্ষেত্রে কোন এফিডেফিট গ্রহণযোগ্য নয়।
